IRS Form 9003-What is it?
Q.I'm reading the required add'l documents to submit with form I-485 and i can't find out (even on the IRS web site) what this form is, did you have to request this from the IRS directly - if so, how long did it take?
A.You can either get this from the INS 1-800 forms number (1-800-870-3676), from your local INS office or from Jonathan's site: http://pages.prodigy.net/alixtcat/forms/irs9003.pdf Note this is an INS form that goes to the IRS not the other way around. My copy of the form is headed "Department of the Treasury--Internal Revenue Service" and the instructions say "This form must accompany your application for permanent residence in the Unites States. ... DO NOT send this form to either of these [Internal Revenue Service and Office of Management and Budget] offices. Instead return it to the appropriate office of the Department of the Immigration and Naturalization Service." It is an IRS form and it does go to the INS - the other way around.
