Does anybody know where (or even if it exists) I can find an Irs form W-9 *.pdf for the '95 tax year?Q.Occasionally, a donor arranges a matching donation to us from his or her employer. Sometimes the employer want us to complete an IRS Form W-9 "Request for Taxpayer Identification Number and Certification". Our ID number appears on the form we send to the employer to confirm that we did indeed receive a donation from the employee. It also appears on our IRS determination letter, which we often send with the confirmation. The instructions for Form W-9 seem to indicate that it is not appropriate in this case. It's primarily for use when making payments that will have to be reported by the payor to the IRS (e.g., for the payor to complete Form 1099). Does anybody know where (or even if it exists) I can find an Irs form W-9 *.pdf for the '95 tax year? I've checked all the sites I could find mentioned here in misc.taxes, but no luck.
